MEDICAL TREATMENT (AMENDMENT) ACT 1997 (NO. 76 OF 1997) - SECT 6

6. The Schedule to the Principal Act is amended—

        (a)     by omitting from Form 1 “ for the condition to which this form relates ”; and

        (b)     by omitting from clause 1 of Form 1 “, in relation to my current condition (describe current condition) ”.
