(1) To obtain redress under the scheme, a person must make an application to the Operator.
(2) To be valid, the application must:
(a) be in the approved form; and
(b) specify where the person lives; and
(c) include any information, and be accompanied by any documents, required by the Operator.
(3) The Operator is not required to make a determination on an
application that is not valid.
