TRANSPORT SAFETY INVESTIGATION (VOLUNTARY AND CONFIDENTIAL REPORTING SCHEME) REGULATION 2012 - REG 12

    The person making the report must include the following information in the report :

  (a)   the name of the person making the report;

  (b)   a description of the reportable safety concern;

  (c)   at least one of the following :

  (i)   the person's postal address;

  (ii)   the person's email address;

  (iii)   the person's telephone number;

  (iv)   the person's fax number.
