SOCIAL SECURITY (ADMINISTRATION) ACT 1999 - SECT 65
Medical certificate in support of claim for sickness allowance
- (1)
- Subject to subsection (3), a claim for sickness allowance must be
supported by the certificate of a medical practitioner which states:
- (a)
- the medical practitioner's diagnosis; and
- (b)
- the medical practitioner's prognosis; and
- (c)
- that the person is incapacitated for work; and
- (d)
- the period for which the person is incapacitated for work.
- (2)
- A certificate for the purpose of subsection (1) must be in accordance with
a form approved by the Secretary.
- (3)
- A person's claim for sickness allowance need not be supported by a medical
certificate if the person is claiming to be qualified for sickness allowance
under section 667 of the 1991 Act.
- (4)
- The Secretary may, in special circumstances, direct that subsection (1) or
- (2)
- does not apply in relation to a claim.
