For subparagraph 3.11 (b) (ii), a provider may apply to the Minister for approval of a bank guarantee.

(2) The application must be in writing and must include the following:

(a)

a copy of the guarantee;

(b)

information showing that the individuals signing the agreement for the provider have the authority to do so;

(c)

information showing that the individuals signing the agreement for the bank have the authority to do so.
