3. After regulation 22 of the Principal Regulations, the following regulations
are inserted: Register of holders of proper authorities

"22A. For the purposes of paragraph 78E (3) (e) of the Act, the information
specified in items 1, 2, 3 and 5 in Form 10C is prescribed. Forms of notice by
licensee to Commission

"22B. (1) A notice under subsection 78F (2) or (3) of the Act must be in
accordance with Form 10A.

"(2) A notice under subsection 78F (4) or (6) of the Act must be in accordance
with Form 10B.

"(3) A notice under subsection 78F (5) of the Act must be in accordance with
Form 10C.".
