(1) A document referred to in an affidavit:
(a) must:

(i)

be attached to the affidavit; or

(ii)

if the document is too large or otherwise unable to be attached — be identified in the affidavit and filed; and

(b)

must bear a statement, signed by the witness before whom the affidavit is made, identifying it as the document mentioned in the affidavit.

Paragraph (1) (b) does not apply to an attachment to an Affidavit of Service (Form 7).
