A party who files a Form 2 must, at the same time, file an affidavit stating the facts relied on in support of the orders sought.

(2)

Subrule (1) does not apply to a Form 2 in which a review of the order of a Judicial Registrar or Registrar is sought.

Note    Some rules require that the affidavit filed with the Form address specific factors (see, for example, rule 5.12).
