An application for an order for substituted service of a bankruptcy notice must be made by filing an application in accordance with the form of application set out in Part 1 of Schedule 2.

(2) The application must be accompanied by:

(a)

a copy of the bankruptcy notice; and

(b)

an affidavit stating the grounds in support of the application.
