(1) The application:

(a)

if made to the Court — must be in accordance with the form of application set out in Part 1 of Schedule 2; or

(b)

if made to a Registrar — must be in accordance with Form 154.

(2)

A single application may be made for the summons of 2 or more examinable persons in relation to a relevant person's bankruptcy.
