Form of proxies
(1)
The appointment of a person as a proxy for a meeting must be by written instrument in accordance with Form 4 in Part 1 of Schedule 4.

(2)
The person appointing the proxy must sign the instrument, or, if incapable of writing, attach his or her mark to it.

(3) The appointment of a person who is blind, or incapable of writing, must not be accepted unless:

(a)
the person attaches his or her signature or mark to the instrument after it has been completed; and

(b)
the instrument is read to him or her by a witness to his or her signature or mark who completes the certificate of witness set out in Form 4.
