PRIVATE HEALTH INSURANCE INCENTIVES REGULATIONS 1998 1998 No. 375 - REG 6

A statement under paragraph 4 (3) (a) or 5 (3) (a):

   (a)  must not include, or be included with, other information that does not
        directly relate to the Government's 30% rebate; and

   (b)  must not accompany other information that does not directly relate to
        the Government's 30% rebate; and

   (c)  must be given to the participant before the end of the financial year
        to which it relates.
