1989 No. 149 TELECOM AUSTRALIA STOCK REGULATIONS - REG 5

5. (1) An application by a member of the public to purchase stock must:

   (a)  be in accordance with the approved form accompanying the prospectus;
        and

   (b)  be made in the manner specified in the prospectus.

(2) In the case of a joint account, the application must set out the names of
the owners in the order in which they are to appear in the Stock Ledger.
