AUTHORITY FOR APPOINTMENT AS A PART-TIME MEMBER OF THE

   (a) 
DIRECTIONS TO VOTER
1. Mark your vote on this ballot-paper by placing the number 1 in the square
opposite the name of the candidate for whom you wish to vote as your first
preference.
2. If you so desire, you may indicate the order of your preference for all or
some of the remaining candidates by placing the numbers 2, 3 and 4 (and so on,
should you so desire) in the squares opposite the names of those candidates.

FURTHER DIRECTIONS TO VOTER
After marking your vote, fold the ballot-paper so as to conceal your vote,
place it in the ballot-paper envelope and seal that envelope. Place the
ballot-paper envelope in the envelope addressed to the Returning Officer, seal
that last-mentioned envelope and complete and sign the declaration printed on
that last-mentioned envelope. Then post the envelope addressed to the
Returning Officer so that it may reach the Returning
Officer not later than 12 o'clock noon on the              day of
19 .

   (a)  Insert the name of the council in relation to which the election is
        held.

   (b)  Full names of candidates to be set out opposite the squares.
