RIGHTS OF THE TERMINALLY ILL ACT 1995 - SECT 17
Certificate of request is evidence
A document purporting to be a certificate of request is, in any proceedings
before a court, admissible in evidence and is prima facie evidence of the
request by the person who purported to sign it or on whose behalf it is
purported to have been signed, for assistance under this Act.
