In all cases where a declaration in lieu of an oath shall have been
substituted by this Act or by virtue of any power or authority hereby given or
where a declaration is directed or authorised to be made and subscribed under
the authority of this Act or of any power hereby given although the same be
not substituted in lieu of an oath heretofore legally taken such declaration
unless otherwise directed by the powers hereby given shall be in the following
form—
‘I A.B. do solemnly and sincerely declare that [let the person declare the
facts] and I make this solemn declaration conscientiously believing the same
to be true and by virtue of the provisions of the Oaths Act 1867 .’.
