PUBLIC HEALTH (INFECTION CONTROL FOR PERSONAL APPEARANCE SERVICES) ACT 2003 - SECT 58

58 Applications

(1) This section applies to the following applications—

(a) an application for a licence under section 30 ;

(b) an application for the renewal of a licence under section 44 ;

(c) an application for the restoration of a licence under section 46A ;

(d) an application for an amendment of a licence under section 47 ;

(e) an application for the transfer of a licence under section 49 .

(2) The application must—

(a) be in the approved form; and

(b) be signed by or for the applicant; and

(c) be accompanied by the fee for the application.
