31 Initial counsellor’s affidavit

The affidavit sworn by the appropriately qualified counsellor who gave counselling to the birth mother, the birth mother’s spouse (if any) and the intended parents (the
"relevant persons" ) must verify a report prepared by the counsellor addressing the matter mentioned in section 22 (2) (e) (ii) , including by stating—

(a) the reasons the counsellor is an appropriately qualified counsellor; and

(b) that counselling about the surrogacy arrangement and its social and psychological implications was given to the relevant persons before the surrogacy arrangement was made.
