(1) This section applies if an insurer is liable for the costs of medical treatment.
(2) Accounts for medical treatment must be sent to the insurer promptly and within 2 months after the treatment is completed.
(3) The accounts must specify—(a) the worker’s full name, date of birth and residential address; and(b) any item number that the medical treatment may have that is listed in the relevant table of costs; and(c) the date of each attendance; and(d) detailed particulars of treatment; and(e) the name and place of business of the worker’s employer.
(4) A worker who receives medical treatment must be given a certificate in the approved form free of charge.
