(1) A notice of appeal must be in the approved form and state—(a) whether the whole or part of the decision is appealed from; and(b) briefly and specifically the grounds of appeal; and(c) the decision the appellant seeks.
(2) If leave is given to start an appeal, the notice of appeal must set out—(a) the order giving leave; and(b) a concise statement of the reasons why leave was given; and(c) the specific questions for which leave was given.
