(1) Section 15 (1) (b) —
omit, insert —

(b) if the appointment is for a health matter—the health care principles and whether the person is likely to apply the principles;

(2) Section 15 (1) (g) , after ‘order’—
insert —
, including whether the person has ever been a paid carer for the adult
