An application for a licence must—
(a) be made to the commissioner; and
(b) be in the approved form; and
(c) be accompanied by—(i) evidence of the applicant’s qualifications and practical experience; and(ii) the fee prescribed by regulation; and(iii) if the applicant is licensed by an interstate or New Zealand licensing authority—written details of any conditions of the licence.
Note—An application for a licence and an application for an endorsement on a licence may be made at the same time. See section 27 .
