(1) The chief executive may, on application by a person, disclose to the person in writing information kept by the chief executive under this Act about an accreditation.
(2) The application—(a) must be in the approved form; and(b) must be accompanied by the written consent of the holder of the accreditation; and(c) may be made by electronic communication.
(3) Subsection (2) (b) does not apply if the applicant is—(a) the holder of the accreditation; or(b) for an accreditation held by a corporation—an executive officer of the corporation; or(c) for an accreditation held by a partnership—a partner in the partnership.
(4) However, if the information is confidential information about an individual, the chief executive must not disclose the information to a person other than the individual without the individual’s written consent.
