65—Application for self administration permit
(1) Subject to
subsection (3), the coordinating medical practitioner for a person may
apply to the Chief Executive for a self administration permit in respect of
the person if the person is physically able to self administer and digest the
poison or controlled substance or the drug of dependence proposed to be
specified in the permit for the purpose of causing the person's death.
(2) An application for
a self administration permit must—
(a) be
in the prescribed form; and
(b)
identify the person in respect of whom the permit is sought; and
(c)
specify the poison or controlled substance or the drug of dependence the
applicant seeks to prescribe or supply in a sufficient dose to the person, for
the purpose of self administration to cause the person's death; and
(d)
specify the contact person in respect of the person; and
(e) be
accompanied by a copy of the completed final review form and all forms
referred to in section 59(1)(a); and
(f) be
accompanied by a statement that the applicant is satisfied of the matters
specified in subsection (3).
(3) The coordinating
medical practitioner must not apply for a self administration permit unless
the coordinating medical practitioner is satisfied that—
(a) the
person has decision making capacity in relation to voluntary assisted dying;
and
(b) the
person's request for access to voluntary assisted dying is enduring.
