152—Particulars required in accident report

For the purposes of section 75(2) of the Act, the following particulars are required:

            (a)         particulars of the vessels involved;

            (b)         the name and address of the owner and the operator of the vessel involved;

            (c)         the time and place of the accident;

            (d)         the circumstances of the accident;

            (e)         the name and address (if known) of each person killed or injured in the accident;

            (f)         the name and address (if known) of each witness to the accident.
