(1)  Following any post-mortem examination, a certificate is to be issued in respect of that examination by the medical practitioner who conducted the post-mortem examination.

(2)  A post-mortem certificate is to be in accordance with Form 4 .

(3)  A medical practitioner may not issue a post-mortem certificate if the medical practitioner is a relative of the deceased person.
