(1)  An application for site approval is to be made to the Minister.

(2)  An application is to contain –

(a) the name and address of the applicant; and

(b) a description of the proposed site.

(3)  An application is to be accompanied by –

(a) a copy of each notice published under regulation 4 ; and

(b) a fee of $300.
