(1)  Subject to regulation 84 , an application for a benefit under these regulations is to be in any form that the Board determines.

(2)  If a person making an application has a spouse or de facto spouse, that person must provide the Board with any documentary evidence that the Board requires in support of the application.

(3)  A person to whom, or in respect of whom, any pension or other benefit under these regulations is being paid is to notify the Board in writing of any change of circumstances that affects or may affect the entitlement of that person to a pension or other benefit under these regulations.
