For the purposes of section 14(3) of the Act, the following are prescribed details:

(a) the full name and honorary title of the person;

(b) the residential or business address of the person;

(c) the veterinary qualifications of the person;

(d) the date on which each qualification was obtained;

(e) the date on which the person was first registered;

(f) each approved speciality to which the registration of the person as a veterinary specialist relates, if the person is to be so registered.
