RADIATION ACT 2005 - SECT 38

An application for an authority must—

        (a)     be in a form approved by the Secretary; and

        (b)     set out which authority is being applied for; and

        (c)     be accompanied by—

              (i)     any information required by the Secretary that relates to the authority; and

              (ii)     the prescribed fee (if any).
