Victorian Consolidated Regulations Victorian Consolidated Regulations(1) For the purposes of section 18AC(1) of the Act, an application must be in Form 12.
(2) For the purposes of section 18AK(1) of the Act, the prescribed form of notice of an application under section 18AC(1) is Form 13.
(3) Subject to section 18AK of the Act, Form 13 must be provided to the court with Form 12, and these Forms may be provided as one document.
