An application under section 244(1) or (3) of the Act must be in writing and include the following details—
(a) the name of the relevant Council;
(b) the address of the relevant rateable property;
(c) the personal details of the occupier making the application;
(d) the name of the property owner;
(e) a declaration signed and dated by the person that states that the person is—
(i) an occupier of the rateable property; and
(ii) liable to pay the rates for that property; and

(iii) not otherwise enrolled on the voters' roll in the municipal district as a resident or ratepayer of a rateable property.
