(1) A person who applies in writing to inspect the register maintained under section 18M(1) of the Act must, in that application, include—
(a) the applicant's full name, address and telephone number; and
(b) a statement that the application to inspect the register is made under section 18M(2) of the Act; and
(c) the name of the Authority whose register is to be inspected; and
(d) the identification of the returns to be inspected by reference to the name of any member or nominated officer whose returns are included on the register; and
(e) the applicant's reason for applying to inspect the register.
(2) An application under sub-regulation (1) must be signed by the applicant and dated.
