For the purposes of section 17C(2)(a) of the Act, the prescribed particulars for an application for a rodeo licence are—
(a) the full name of the applicant; and
(b) if the applicant is a body corporate, the name, registered address and an ABN; and
(c) the street and postal address of the applicant; and
(d) a contact telephone number and email address (if any) for the applicant; and
(e) unless the applicant is an approved rodeo organisation, the name of the approved rodeo organisation that the applicant is accredited with; and
(f) an animal welfare plan that includes—
(i) a description of the role or roles that any person employed to assist the applicant will undertake to monitor or discharge requirements under this Part or Division 2 of Part 2 of the Act; and
(ii) a description of the reporting and communication processes that will be implemented by the applicant, and any person employed to assist the applicant, and the nominated veterinary practitioner to support compliance with this Part or Division 2 of Part 2 of the Act; and
(g) the date that the form was signed by the applicant.
