(1) If —
(a) a
certificate has been given under section 38(1) in relation to an analysis or
examination; and
(b) an
accused applies to the Commissioner for a copy of the certificate,
the Commissioner shall
as soon as practicable cause a copy of the certificate to be provided to the
accused.
(2) An application
shall be in writing in the prescribed form and shall specify the address at
which a copy of the certificate can be provided to the accused.
