(1) If an authorised
officer reasonably believes that a person has a notifiable infectious disease,
the authorised officer may require the affected person to give the authorised
officer the following information —
(a) the
affected person’s name, residential address, telephone numbers, email
address, date of birth and gender;
(b)
information about any circumstances in which the affected person may have
acquired, or been exposed to, the notifiable infectious disease;
(c)
information about any circumstances in which the affected person may have
exposed another person to the notifiable infectious disease;
(d) the
name, residential address, telephone numbers, email address, date of birth and
gender of any person the authorised officer reasonably believes is a contact
person in relation to the affected person;
(e) any
other information prescribed by the regulations.
(2) The information
prescribed by the regulations for the purposes of subsection (1)(e) may vary
in respect of different notifiable infectious diseases.
