(1) In this section
—
cause of death certificate means a certificate of
the cause of a person’s death under the
Births, Deaths and Marriages Registration Act 1998 section 44(1).
(2) The coordinating
practitioner or administering practitioner for a patient must, within 2
business days after becoming aware that the patient has died (whether or not
after self-administering, or being administered, a voluntary assisted dying
substance in accordance with this Act), notify the Board, in the approved
form, of the patient’s death.
(3) Subsection (2)
does not apply if the administering practitioner for a patient gives the Board
a copy of a practitioner administration form in respect of the patient under
section 61(4).
(4) Subsections (5)
and (6) apply if a medical practitioner who is required to give a cause of
death certificate for a person knows or reasonably believes that the person
was a patient who self-administered, or was administered, a voluntary assisted
dying substance in accordance with this Act.
(5) The medical
practitioner must, within 2 business days after becoming aware that the person
has died, notify the Board, in the approved form, of the person’s death,
unless the medical practitioner is the coordinating practitioner or
administering practitioner for the person.
(6) The medical
practitioner must not include any reference to voluntary assisted dying in the
cause of death certificate for the person.
