(1) An application for
the purposes of this Part must be —
(a) made
in the form approved by the Chief Health Officer or, if no form is approved,
in writing; and
(b)
accompanied by the scheduled fee.
(2) An applicant must
provide the Chief Health Officer with any document or other information
relating to the application that the Chief Health Officer asks for.
(3) If an applicant
does not comply with the requirements of this Part for the making of the
application, the Chief Health Officer may decline to deal with it and must
advise the applicant accordingly.
