(1) Except as provided
in regulation 27, an application for a permit must —
(a) be
made in a form and in the manner approved by the CEO; and
(b)
provide the information required by the form; and
(c) be
lodged with the CEO together with any application fee prescribed under these
regulations.
(2) The CEO may
require an applicant to provide additional information reasonably related to
an application before determining the application.
(3) The CEO may refuse
to consider an application which is not in accordance with
subregulation (1) or where an applicant has not complied with
subregulation (2).
