STATE SUPERANNUATION REGULATIONS 2001 - REG 251

251 .         Form of documents and information

        (1)         Except as provided in regulations 47A(5A), 69J(2), 79A(6), 111D(2) and 123(5A), an application, approval, certificate, determination, direction, notice, notification, report, request, return or statement made or given for the purposes of the Act must be made or given in writing.

        (2)         If a person is required by these regulations to provide information or evidence to the Board, the Board may require the person to provide that information or evidence in writing and the person is to comply with that requirement.

        (3)         A document to be used for the purposes of the Act must be —

            (a)         in the form (if any) determined by the Board; and

            (b)         signed by the person making or giving it; and

            (c)         if it is to be given to the Board, delivered to the offices of the Board.

        (4)         The Board may accept a document given to it —

            (a)         other than in accordance with subregulation (3); or

            (b)         after the expiry of the time fixed under the Act for giving the document.

        (5)         In this regulation —

        document means anything in writing.
