(1) The additional
matters that are to be included in a notification of a payment of unclaimed
money (or prescribed retained money) are —
(a) the
name and address of the person who made the payment; and
(b) the
address of the place at which the money was handed over or to which it was
sent.
(2) A notification of
a payment of unclaimed money or prescribed retained money is to be in the form
of Form 2 in Schedule 1.
