(1) An application for
registration of the design of an item of plant must be made in the manner and
form required by the regulator.
(2) The application
must include the following information —
(a) the
applicant’s name;
(b)
whether or not the applicant is a body corporate;
(c) if
the applicant conducts the business or undertaking under a business
name — that business name and a certificate or other written
evidence of the registration of the business name;
(d) any
other evidence of the applicant’s identity required by the regulator;
(e) a
statement signed by the designer of the item of plant —
(i)
stating that the designer has complied with the
designer’s obligations under section 22 of the Act in relation to
the design; and
(ii)
specifying the published technical standards and
engineering principles used in the design;
(f) a
design verification statement that accords with regulation 251;
(g)
representational drawings of the design;
(h) a
declaration that the applicant does not hold an equivalent registration under
a corresponding WHS law.

(3) Any drawings or
other documents provided with the application must be capable of being kept in
an electronic form.
(4) The application
must be accompanied by the relevant fee.
