Every person, being
the holder of a licence for the commencement, construction, enlargement,
deepening or alteration of an artesian well, shall, at the expiration of each
calendar month after the day of issue of the licence, until the completion of
the work, and within 30 days after the completion of the work, forward to
the Commission a statement regarding that well in the form of Form 4 in the
Schedule 1.
