REGISTRATION OF IDENTITY OF PERSONS REGULATIONS 1975 - REG 3

3 .         Application for entry of identity (Form 1) (Form 2)

                An application to the Registrar General for the entry of a person’s identity in the Register pursuant to subsection (3) of section 4 shall — 

            (a)         in the case of a person who has attained the age of eighteen years, be in the form of Form 1; and

            (b)         in the case of a person who has not attained the age of eighteen years, be in the form of Form 2.
