(1) The Board must issue a certificate of registration to a person who is granted registration.(2) A certificate of registration is to (a) be in such form and contain such information as the Board determines; and(b) specify the conditions, if any, to which the registration is subject.(3) A certificate of registration is evidence that, for the period or until the date specified in the certificate (a) the person named in the certificate is a registered practitioner; and(b) the person named in the certificate is entitled to practise in this State subject to the conditions, if any, specified in the certificate.(4) If the Board is satisfied that a certificate of registration has been lost or destroyed it may issue a replacement certificate, marked as such, on payment of the prescribed fee.(5) The Board, by notice, may require a registered practitioner to surrender his or her certificate of registration to enable the Board to issue that practitioner with a new certificate of registration with amended particulars.(6) A registered practitioner who is given a notice under subsection (5) must surrender his or her certificate of registration to the Board within 14 days after receiving the notice or within such longer period as the Board may allow.Penalty: Fine not exceeding 15 penalty units.
