SUPREME COURT (MISCELLANEOUS CIVIL PROCEEDINGS) RULES 2018 - REG 14.14

Application for change of name on the Supreme Court roll

    (1)     A lawyer whose name is on the Supreme Court roll may apply to the Court for the lawyer's name to be changed on the Supreme Court roll.

    (2)     The application shall—

        (a)     be in Form 2–14A; and

        (b)     be accompanied by an affidavit in support setting out the reasons for the change of name and exhibiting any relevant supporting documentation.
