(1) The Board is
required to keep an accurate and up-to-date register of registered pharmacies
in such manner and form as the Board determines.
(2) The following
information is to be recorded in the register in respect of a registered
pharmacy —
(a) the
address of the pharmacy;
(b) the
name of the person in whose name the pharmacy is registered;
(c) any
conditions imposed on the registration of the pharmacy;
(d) such
other information, if any, as is prescribed by the regulations.
