1986 No. 150 FUTURES INDUSTRY REGULATIONS - REG 17

17. (1) For the purposes of sub-section 65 (4) of the Act, the prescribed form
of application under sub-section 65 (1) of the Act for a futures broker's
licence or a futures adviser's licence is-

   (a)  Form 3, if the applicant is a natural person; or

   (b)  Form 4, if the applicant is a body corporate.

(2) For the purposes of sub-section 65 (4) of the Act, Form 5 is the
prescribed form of application under sub-section 65 (1) of the Act for a
futures broker's representative's licence or a futures adviser's
representative's licence.
