3. (1) A form must be completed in accordance with a direction specified in,
or at the foot of, the form.

(2) A person who is required for the purposes of the Act or these Regulations:

   (a)  to complete a form; or

   (b)  give notice or make a report; must complete the form, give notice or
        make the report in sufficient detail to allow proper consideration of
        the form, notice or report.

(3) A form, notice or report must be produced clearly and legibly in
handwriting or by means of a machine in such a manner as to enable clear and
legible reproduction of the contents of the form, notice or report.
