(1) The application
must —
(a) be
made in accordance with the approved form; and
(b) be
made in the approved manner; and
(c)
include details of —
(i)
the activities carried out by the applicant under the
licence during its current term; and
(ii)
the amount of money spent by the applicant in relation to
the blocks covered by the licence during its current term; and
(iii)
the activities that the applicant intends to carry out
under the licence during the term applied for; and
(iv)
the amount of money that the applicant intends to spend
on those activities during the term applied for;
and
(d)
specify the blocks that the applicant nominates for surrender in accordance
with section 104.

(2) The applicant may
include in the application any other information that the applicant thinks is
relevant.
